Physical Therapist (1 Position Available)

Duties and Responsibilities:

Work with a full range of patients to provide care in the comfort of their own home.
Establish and execute treatment plans along with the members of our team, while adhering to healthcare standards.
Evaluate and treat patients suffering from physical disability or lack of mobility due to injury, disease, or surgery in a home care environment.
Establish and administer a treatment plan for patients’ pain management and mobility improvement.
Serve as partners of patients and their family members in rehabilitative care and independent living methods and techniques.

QUALIFICATIONS:

One (1) year of professional experience practicing as a Physical Therapist (PT) in a home health or similar setting.
Licensure: Current, unrestricted license to practice as a Physical Therapist (PT) in the State of CA.
Current CPR/AED/BLS/First Aid certification.
Knowledge of Electronic Medical Record.
Reliable transportation.

We also have OPEN POSITIONS for our INTERMEDIATE CARE FACILITIES for the DEVELOPMENTALLY DISABLED-HABILITATIVE, in Hayward, CA.

HOUSE MANAGER (1 Position Available)

Duties and Responsibilities:

Overseeing daily operations, assist with budgeting and implementing programs for residents.
Assists with hiring and training of new employees.
Identifies the training needs of group home staff, provide coaching/mentoring to improve their knowledge or skills as required.
Assist the Administrator with staff’s monthly schedule, meetings, resident reviews, appointments, and other program related activities.

QUALIFICATIONS:

At least 12 months of work experience or related experience in caring for the developmentally disabled
Direct Support Professional Training – will train once hired
Current CPR and First Aid certification.
Strong Leadership and Organization Skills.
Minimum of High School Diploma, GED. Higher education a plus but not required.

DIRECT CARE STAFF (11 POSITIONS AVAILABLE)

Duties and Responsibilities:

Provide direct care to residents with developmental disabilities.
Assist patients with day-to-day activities.
Meal Planning and Food Preparation
Medication and Hygiene Management.
Assist in data collection and training of residents.

QUALIFICATIONS:

At least 6 months of work experience in caring for the developmentally disabled.
Current CPR and First Aid certification.
Minimum of High School Diploma, GED. Higher education a plus but not required
